In-Person visits: There are stairs up from the parking lot. Come into the building and we are on your left. If you would like to use the elevator, park towards the north end of the lot and go into the door off the parking lot. Walk around the stair case to the elevator. You will then walk down the long hall to our office.
TELEHEALTH visits: ONLINE only (must be in Illinois).
